ประสิทธิผลของโปรแกรมจัดการรายกรณีแบบ SMART Care ต่อความร่วมมือในการรักษาของผู้ป่วยเอชไอวี คลินิกนภา โรงพยาบาลบึงกาฬ

การศึกษานี้เป็นการวิจัยทดลองเบื้องต้นแบบกลุ่มเดียว วัดผลก่อน-หลัง เพื่อศึกษาประสิทธิผลของโปรแกรมจัดการรายกรณีแบบ SMART Care ต่อความร่วมมือในการรักษา ความสม่ำเสมอในการรับประทานยาต้านไวรัส ระดับไวรัสในเลือด และระดับ CD4 ของผู้ป่วยเอชไอวี กลุ่มตัวอย่างคือผู้ป่วยเอชไอวีที่มีระดับไวรัสในเลือดมากกว่า 50 copies/mL จำนวน 36 คน รักษาที่คลินิกนภา โรงพยาบาลบึงกาฬ เครื่องมือที่ใช้ คือโปรแกรมจัดการรายกรณีแบบ SMART Care ประกอบด้วย (1) การสนับสนุนการจัดการตนเอง (S) (2) ระบบติดตามเฝ้าระวัง (M) (3) ระบบแจ้งเตือนผ่านแอปพลิเคชัน (A) (4) ทีมดูแลที่ตอบสนองเร็ว (R) และ (5) การใช้เทคโนโลยี (T) ดำเนินกิจกรรมทั้งหมด 6 ครั้ง ระหว่างเดือนมกราคมถึงมิถุนายน พ.ศ. 2568 เก็บข้อมูลด้วยแบบสอบถามข้อมูลทั่วไป แบบสอบถามความร่วมมือในการรักษา แบบบันทึกการรับประทานยา และแบบบันทึกผลตรวจทางห้องปฏิบัติการ วิเคราะห์ข้อมูลด้วย paired t-test และ Wilcoxon signed-ranks test ผลการศึกษาพบว่า หลังการทดลอง ความร่วมมือในการรักษาโดยรวมเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p<0.05) ทั้งในด้านการรับประทานยาต้านไวรัส ด้านการมาตรวจตามนัด และด้านการปฏิบัติตามคำแนะนำ (p<0.05) ความสม่ำเสมอในการรับประทานยาต้านไวรัสเพิ่มขึ้นจากร้อยละ 91.99 เป็นร้อยละ 97.15 ระดับไวรัสในเลือดลดลงอย่างมีนัยสำคัญทางสถิติ (p<0.05) โดยผู้ป่วยทุกรายมีระดับไวรัสน้อยกว่า 50 copies/mL ส่วนระดับ CD4 ไม่แตกต่างกัน (p>0.05) สรุปได้ว่า โปรแกรมจัดการรายกรณีแบบ SMART Care มีประสิทธิผลในการส่งเสริมความร่วมมือในการรักษาและความสม่ำเสมอในการใช้ยา ส่งผลให้การควบคุมไวรัสดีขึ้นอย่างมีนัยสำคัญ จึงเหมาะสำหรับการประยุกต์ใช้ในการดูแลผู้ป่วยเอชไอวี
